How to best prepare for your arrival
We know that moving into a care unit represents a significant milestone in your life or that of a loved one, and that it comes with its share of questions. To help you navigate this new state, we’ve put together a guide that answers some of the most common aspects about arriving at a care unit. Above all, remember that you’re not alone. Our dedicated teams are here to support you every step of the way, ensuring a smooth and comfortable life change. We wish you a warm welcome and a gentle settling-in process.
1st step: Introduction to our service offering and living environment
Our Rental Manager will take the time to get to know you and understand your needs, providing personalized guidance.
• Schedule a tour of our residence and care unit, and let us show you around!
2nd step: Gathering essential documents for your assessment
To complete your health needs evaluation, we require some essential documents. Please note that there may be a delay in receiving this information, so it’s crucial to act quickly to avoid any setbacks.
• A summary of your medical history and current medications;
• A list of your support network, if applicable (social worker, primary nurse, etc.).
3rd step: Visiting the residence and care unit
There’s no better way to get a feel for a place than to visit in person and meet the people who live and work there. During your visit, you’ll be accompanied by a member of our leasing team, and we’ll most certainly introduce you to the Director of Nursing (DN).
4th step: Personalized evaluation by the Director of Nursing
To develop a care plan tailored to your requirements, the Director of Nursing (DN) will conduct a comprehensive assessment of your health needs.
• A private consultation with the Director of Nursing (feel free to bring a family member or friend for support);
• If you haven’t received your medical history summary before your visit, simply forward it to us later to complete your evaluation.
5th step: Personalized health needs assessment and cost review
The Director of Nursing (DN) and the Rental Manager will present your personalized assessment and associated costs. This meeting can be held at the residence or via video conference, at your convenience. Please note that the assessment is dependent on us receiving your medical history summary*.
• The Director of Nursing (DN) and the Rental Manager will answer all your questions.
*Admission to the care unit depends on the match between your health assessment and the services we offer.
6th step: Reserving your apartment
The Director of Nursing (DN) will guide you through the final steps to prepare for your move-in.
• Choosing your unit;
• Confirming your move-in date;
• Creating a list of personal items to pack and bring with you.
7th step: Signing your lease
We’ll invite you to the residence to sign your lease agreement.
• Please note that a first month’s rent payment will be due at this time.
8th step: Moving in
It’s the big day! Our team will be waiting to greet you and help you settle into your new living space.
• Please arrive at the scheduled time at the main entrance;
• Your Rental Manager will escort you to your apartment;
• Our care team, led by the Director of Nursing (DN), will be on hand to ensure a warm and supportive welcome.
